Khomeini's revolution was run by the PLO, a faction of the Muslim Brotherhood that was playing both sides throughout the Cold War going back to WWI when they worked for the Germans during the war, then after the war the British made them the leaders of Palestine's Muslims, then they worked for the Germans again during WWII, then the CIA and UK recruited them when taking over Germany's intel networks after the war, the CIA trained them and oversaw their first attacks on Israel's water supply, then LBJ cut them loose so they went to Asia and picked up support from North Vietnam, China, and Russia. The Russians made up the story that they were the Palestinian People and their Sole Legitimate Representative, and sponsored their attempts to overthrow pro-Western governments in Jordan and Lebanon.

When the Iranian revolution happened, most of the West's analysts thought that it was a good thing that would liberalize Iran. So Western intelligence was full of idiots and probably a large number of traitors taking orders from Moscow. It is possible that somebody might have thought that the Shah was an embarrassment, mistakenly thought that the PLO were assets, and deployed them. I wouldn't know, however.
